• Transcript of Records (TOR)
• 3 character references (with position / affiliation) and contact numbers
• Employment Certificate from previous employers
• Recommendation letter from previous employer / college dean or Professor
• Two 2 x 2 pictures (white background)
• TIN, SSS and NBI Clearance
Others:
• Results of most recent physical examination
• Birth Certificate
• Barangay Certificate of Residency
B. For teacher Aides and non-academic staff
The applicant needs to submit the following:
• Resume or Bio-Data
• Birth certificate
• Barangay certificate of residency
• Results of physical examination
• Two 1 x1 pictures (white background)
• SSS, TIN, NBI clearance
• Employment certificate from previous employers
